2. E-studio Photo A4 printed sheet with QR code
3. Birth Certificate (took within 6 Months) can get it DS office for 100/=
4. Previous/Old NIC, If it’s lost need to attach police report
5. Envilop with (Register Post price) Stamp
6. 250/= fee pay at GS office and paste the receipt on form.
Can be hand over to GS.
