1. This form needs to be filled only in ENGLISH .
2. If any additional details needs to included you may attach separate sheet mentioning the row number . Eg Row 22.
3. Applications will be rejected if the below mentioned certifications are not complied with .
4. Copies required in rows numbers 15 , 21 , 23 , 27, 31 , 33 , 34 , 35 are COMPULSORY and needs to be uploaded in the google forms .
5. Certification by the applicants current school principal is COMPULSORY and the certification needs to be uploaded in to this google forms.
6. Member YMMA certification is COMPULSORY and the certification needs to be uploaded in to this google forms.
7. In case there is no Member YMMA in your area certifying by your area Masjid is COMPULSORY and it needs to be uploaded in to to this google form .
8. All documents which needs to be uploaded ONLY ON PDF FORMAT.
9. The applicant has to have a minimum result of 5 A s in the GCE O/L examination .
10. In the above mentioned certifications make sure the the seal is placed and the form is properly filled , incomplete forms will be rejected .
